Problembeschreibung:&#039;&#039;&#039; <br /> <br /> Stationäre Aufnahme bei akuter Lymphangitis mit Verdacht auf ein infiziertes Hämatom linker Oberschenkel. Es bestand ein Z. n. Prellung mit Hämatom unter Markumartherapie mit Hämatomausräumung in einem vorherigen stationären Aufenthalt.<br /> Im jetzigen stationären Aufenthalt erfolgte einen Tag nach Aufnahme die Revision, Abtragung von nekrotischem Gewebe und Einlage von Refobacin-Ketten. Bei der weiteren Präparation während der OP zeigte sich massiv nekrotisches Subcutangewebe bis zur Fascia lata. Es wird eine großzügige Nekrosektomie durchgeführt mit gründlicher Spülung. Anschließend wird die gesamte Fascia lata mit dem scharfen Löffel abgekratzt. Entfernen des gesamten nekrotischen Gewebes. <br /> Da die operative Prozedur eine Nekrosektomie der Faszie erfolgt ist, stellt sich hier die Frage nach der spezifischeren Beschreibung der Hauptdiagnose. Der MDK favorisiert die L02.4. Seitens unserer Klinik wurde die M72.65 als HD kodiert.<br /> <br /> &#039;&#039;&#039;2. Frage:&#039;&#039;&#039; <br /> <br /> Ist als Hauptdiagnose die L02.4 Hautabszess, Furunkel und Karbunkel an Extremitäten oder die M72.65 Nekrotisierende Fasziitis Oberschenkel (mit Erreger B95.6!
