Adsorptionsverfahren ein eigenständiges therapeutisches Verfahren darstellt und wegen abweichender Technologie und Behandlungszielen methodisch in keiner Weise mit anderen extrakorporalen Verfahren (z.B. Hämofiltration, Hämodialyse, Hämodiafiltration, ECMO, Herz-Lungen-Maschine) gleich zu stellen ist. Daraus folgt, dass eine Ablehnung der Kodierung und Abrechnung des o.g. Adsorptionsverfahren seitens der MDK bzw. Krankenkassen auf Grundlage des Grundsatzes der monokausalen Kodierung nicht korrekt ist.<br /> Erläuterung: Die Adsorption hydrophober, niedrig- und mittelmolekularer Substanzen („CytoSorb-Adsorber“, Hersteller CytoSorbents Inc.)) stellt ein eigenständiges therapeutisches Verfahren dar und ist wegen abweichender Technologie und Behandlungszielen methodisch in keiner Weise mit anderen extrakorporalen Verfahren (z.B. Hämofiltration, Hämodialyse, Hämodiafiltration, ECMO, Herz-Lungen-Maschine) gleich zu stellen. Bei dem o.g. Adsorptionsverfahren steht die Entfernung von nicht wasserlöslichen, proteingebundenen Substanzen mittels extrakorporaler Adsorption aus dem Vollblut im Vordergrund, die mittels sonstiger extrakorporaler Verfahren nicht oder nicht adäquat entfernt werden können. Die extrakorporale Membranoxygenierung (ECMO) zielt auf die Unterstützung der Sauerstoffzufuhr und Entfernung des Kohlendioxyds sowie – je nach Aufbau - einer Unterstützung des Kreislaufes ab und wird nicht selten in Situationen angewandt, in denen zeitgleich eine systemische Hyperinflammation gegeben ist, welche durch die CytoSorb-Therapie adressiert werden soll. Die ähnlich der ECMO aufgebaute Herz-Lungen-Maschine (HLM) dient dem Ersatz der Lungen- und Herzfunktion im Rahmen von herzchirurgischen Eingriffen. CytoSorb kann hier helfen – vor allem bei komplexen Eingriffen mit längerer Maschinenzeit, hyperinflammationsbedingte Komplikationen zu vermeiden. Alternativ kann der intraoperative Einsatz des Adsorbers an der HLM bei herzchirurgischen Notfalleingriffen an Patienten mit vorbstehender Ticagrelor-Therapie (Plättcheninhibtor) zur Vermeidung von Blutungskomplikationen sinnvoll sein.
