Problembeschreibung:&#039;&#039;&#039;<br /> <br /> Aus der Sicht des Chirurgenteams ist die radikale Lymphknotendissektion nur in den europäischen Guidelines klar formuliert und besagt, dass 3 mediastinale Lymphknotenstationen beteiligt sein müssen (obligat ist dabei die Station 7, bzw. infracarinal) und weitere Lymphknoten 10-14, sodass insgesamt mindestens 6 Lymphknoten entnommen werden müssen. <br /> <br /> Aus der Sicht meines Controllers ist es jedoch keine radikale Lymphadenektomie. <br /> Diese wäre bei Ausräumung von mindestens 6-8 Stationen gegeben. <br /> Diese Aussage ist jedoch (aus meiner Sicht) nirgends belegt und eher eine Bauchentscheidung.<br /> <br /> In manchen Fällen ist es jedoch erlösrelevant, ob eine regionale Lymphadenektomie oder eine radikale Lymphadenektomie kodiert wird.<br /> Im OPS Verzeichnis gibt es zu der Anzahl der Lymphknotenstationen keinen Hinweis/ keine Definition! <br /> <br /> Bsp.:<br /> Für die Abbildung einer Lymphknotendissektion im Rahmen einer Mediastinoskopie oder Thorakoskopie werden folgende OPS verwendet:<br /> <br /> - 5-401.- Exzision einzelner Lymphknoten und Lymphgefäße <br /> - 5-402.- Regionale Lymphadenektomie (Ausräumung mehrerer Lymphknoten einer Region) als selbstständiger Eingriff <br /> - 5-404.- Radikale (systematische) Lymphadenektomie als selbstständiger Eingriff <br /> <br /> Die Differenzierung zwischen einer regionalen und radikalen Lymphadenektomie wäre für die Thoraxchirurgie obligat, da die Angabe in vielen Eingriffen vorausgesetzt wird. <br /> Sei es bei einer: <br /> Lobektomie: 5-324.-<br /> Segmentresektion: 5-323.4-<br /> Keilresektion: 5-322.-<br /> <br /> <br /> <br /> &#039;&#039;&#039;3.
