Es erfolgte eine geschlossene Reposition und Osteosynthese. Bei der Patientin bestand eine alkoholtoxische Leberzirrhose und in der Vorgeschichte wurde ein Mammakarzinom erwähnt. Der Zustand der Patientin verschlechterte sich postoperativ. Zudem bestand, kurz bevor die Patientin verstarb, der Verdacht auf eine Pneumonie. Es wurde geröntgt und eine Pleurasonographie durchgeführt. Eine antibiotische Behandlung wurde begonnen. Eine Obduktion wurde durchgeführt. Hier zeigte sich kein Hinweis auf eine Pneumonie, sondern eine beginnende Lymphangiosis carcinomatosa der Lunge, histologisch vereinbar mit einem Mammakarzinom. Ist die Kodierung der nachfolgenden Nebendiagnosen sachgerecht? J18.9 Pneumonie, nicht näher bezeichnet, C78.0 Sekundäre bösartige Neubildung der Lunge.<br /> <br /> == Kodierempfehlung SEG-4: ==<br /> Im Rahmen einer Obduktion festgestellte Diagnosen, die aus retrospektiver Sicht während des stationären Aufenthaltes das Patientenmanagement beeinflusst haben, können kodiert werden, auch wenn die Diagnosebestätigung oder -stellung erst im Rahmen einer Obduktion erfolgt. Demzufolge ist nur die Kodierung von C78.0 Sekundäre bösartige Neubildung der Lunge sachgerecht.
